Air-Fryer Cooking Trial Design to Lower Blood Pressure
Design a randomized controlled trial to test whether replacing conventional frying with air-fryer cooking affects blood pressure.
Sample size and trial arm structure
Calculate sample size using a prespecified primary outcome of change in systolic blood pressure over the intervention period, selecting a minimally clinically important difference (for example 3 to 5 mmHg) and an estimate of standard deviation from prior dietary or lifestyle BP trials; set power at 80 to 90 percent and two-sided alpha at 0.05, then inflate the number for expected dropouts and nonadherence. Choose a parallel-group design when anticipating persistent dietary carryover or when the intervention period must be long to observe effects, and choose a randomized crossover design only if the expected intervention effects are acute, reversible, and a sufficiently long washout period can be implemented to avoid carryover; if a crossover is used, calculate within-subject variance to inform sample size rather than between-subject variance. Randomize using a concealed, computer-generated sequence and stratify by key variables that influence blood pressure such as baseline hypertensive status, antihypertensive medication use, and sex to ensure balanced arms.
Dietary control, intervention delivery, and adherence monitoring
Provide a tightly controlled dietary protocol by specifying which foods are prepared via air-fryer versus conventional methods, standardizing portion sizes, sodium content, meal timing, and cooking oils so the only intended variable is the cooking method; supply standardized recipes, measured ingredients, and, when feasible, provide prepared meals or raw ingredients to participants to minimize variation. Monitor adherence objectively through a combination of methods such as weighed food returns, time-stamped photographs of meals with participant identifiers, and device usage logs from smart air-fryers or plug-in power meters, and define prespecified adherence thresholds that trigger sensitivity analyses or per-protocol analyses; collect and log deviations in a case report form to allow rigorous assessment of fidelity. Include safety instructions about proper handling and cleaning of air-fryer equipment, avoidance of overheating or placing nonapproved materials in the appliance, explicit guidance for participants with known food allergies, and a protocol for reporting adverse events such as burns, foodborne illness, or changes in medication, with an identified clinical contact available throughout the trial.
Blood pressure measurement and mechanistic biomarker endpoints
Measure blood pressure using validated automated devices with standardized procedures: have participants rest seated for five minutes, avoid caffeine, exercise, and smoking for 30 minutes prior, take at least three measurements one minute apart and use the average of the last two as the clinic BP; complement clinic measurements with 24-hour ambulatory blood pressure monitoring at baseline and end of intervention to capture nocturnal and daytime patterns and to reduce white coat effects. Include mechanistic biomarker endpoints that can clarify pathways linking cooking method to blood pressure changes, such as plasma markers of oxidative stress, inflammatory cytokines, circulating lipid oxidation products, and urinary sodium and potassium to verify dietary intake; collect fasting blood and timed urine samples at baseline and follow-up visits, store samples under standardized conditions, and predefine laboratory assays and quality control procedures. Prespecify primary and secondary analyses including intention-to-treat and per-protocol populations, adjust for baseline BP and relevant covariates in statistical models, and include safety monitoring with predefined stopping rules for serious adverse events.
